APPLICATION / REGISTRATION FORM

A candidate seeking registration to the Uttar Pradesh Common Admission Test - DENTAL (UPCAT- DENTAL) is required to send his / her application on the prescribed application form only, supplied with the Information Brochure. A candidate must not fill & submit more than one form for the examination.

The Information Brochure containing application form for registration for entrance examination and admission in B.D.S. course for the Academic Year 2008-09 will be available from  member institutes.

The application form can be obtained  in person against a crossed bank demand draft of Rs. 600/- (Rs. Six Hundred Only) (Not by cash)  drawn in favour of The Association of Private Medical & Dental Colleges of Uttar Pradesh payable at Bareilly.
